Employee Benefits

We provide all eligible employees with single medical, prescription, and dental coverage. All full-time employees are required to sign up for coverage because our rates are based on the number of employees on our plans.

Your coverage will be effective the first day of the month following your start date. If your start date is on the first day of the month, insurance coverage will begin that day.

Marriage, divorce, or legal separation
●    Death of spouse or other dependent
●    Birth or adoption of a child (60 days)
●    A spouse’s employment begins or ends
●    A dependent’s eligibility status changes due to age, student status, marital status, or employment
●    You or your spouse experience a change in work hours that affects benefits eligibility
●    Relocation into or outside of your plan’s service area
●    Loss of eligibility under Medicaid or CHIP (60 days)
●    Eligibility for premium assistance under Medicaid or CHIP (60 days)

Every year in April or May, WDMCS has a two-week period where you can sign up for your medical, prescription drug, and other benefits. You have a chance to make changes like switching your medical plan, adding or removing dependents, and adjusting your supplemental life insurance. Remember to do this every year!

• Visit the American Fidelity AFenroll website.
• Type in your username or social security number.
• Type in your Personal Identification Pin (PIN). You can reset your pin later if you forget it.
• Click "Log in."
   

A two-factor authentication pop-up window will appear. Select the email or text number for the code to be sent to you. Once you receive the code through email or text message, type it into the account code box. You should then be able to access the list of benefits you have enrolled in for the current year. 

If you have never logged in and set up security questions, your initial PIN will be the last four digits of your social security number (SSN) and the last two digits of your birth year. For example, if your SSN is  ***-**-1234, and you were born in 1980, the default PIN would be 123480. You will then be able to reset your PIN to whatever you choose.

We provide all eligible employees with single dental coverage. All full-time employees are required to sign up for coverage because our rates are based on the number of employees on our plans.

The Delta Dental PPO plan allows you to visit any dentist that you want. However, when you use a PPO dentist, you will have lower out-of-pocket expenses. This is due to the fact that PPO dentists have agreed to special pricing arrangements with Delta Dental. As a result, your coinsurance amounts will be calculated off of a lower amount. Benefit period deductible and maximum are calculated on a calendar year basis.

Contact lenses or frames are once within a 12-month period defined by the last date of service.

Frames are once within a 24-month period defined by the last date of service.

Exams are covered by the Wellmark Medical plan.
