Login Instructions for Benefits
Login Instructions to Review Benefits
- Visit the American Fidelity AFenroll website.
- Type in your username or social security number.
- Type in your Personal Identification Pin (PIN). You can reset your pin later if you forget it.
- Click "Log in."
A two-factor authentication pop-up window will appear. Select the email or text number for the code to be sent to you. Once you receive the code through email or text message, type it into the account code box. You should then be able to access the list of benefits you have enrolled in for the current year.
If you have never logged in and set up security questions, your initial PIN will be the last four digits of your social security number (SSN) and the last two digits of your birth year. For example, if your SSN is ***-**-1234, and you were born in 1980, the default PIN would be 123480. You will then be able to reset your PIN to whatever you choose.

Transparency in Coverage
The link below leads to the machine readable files that are made available in response to the federal Transparency in Coverage Rule and includes negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. The machine-readable files are formatted to allow researchers, regulators, and application developers to more easily access and analyze data.
