2010 - 2011 Emergency Communication Plan Please complete a form for EACH student. This document will be kept with classroom teachers to assure us that all students have a plan to arrive home safely when school is dismissed early due to an emergency situation including but not limited to:
Hazardous weather conditions
A community safety concern
Loss of water or power at our school for an extended time.
2010 - 2011 Volunteer Driver and Liability Form. This document must be completed by ANYONE who using their own vehicle to transport students for school trips. When returning the form to school, please remember to include a copy of your current driver's license and automobile liability insurance card.
2010 - 2011 Release of Information Form. This document is to be completed if you do not wish to have your child's photo released for educational purposes. The West Des Moines Community School District often uses student photographs and/or names in school/district publications and websites. Please note: no names are included with webpublished photographs. In addition, no personal information such as address/phone number/photograph is included with web-published names. If you approve the district using photographs and/or your child’s name in publications and onwebsites, you do not need to return this form.

At Jordan Creek we believe in maintaining an environment that supports a healthy lifestyle. By educating our students about the importance of daily physical activity through recess and PE classes and also how to grow healthy bodies by making smart food choices, our goal is to help you raise a child that is healthy and happy! To support this important initiative, we ask that when sending treats to school for birthdays and other celebrations, you carefully choose a treat that will honor our mission. Please see attachment for more information and a list of healthy alternatives to not-so-healthy foods.”
Please remember when packing school lunches and sending treats to school, that Jordan Creek continues to be a peanut-restricted school. We have students who have a LIFE-THREATENING allergy to peanuts and peanut products, and even the mere odor of peanut products can trigger a severe situation for them. Many pre-packaged food items have cautionary information printed on the label. If you have any questions, please call the school nurse, Lisa Farrell, at 633-5223.
A Letter from the Principal Dear Students and Families,
We wish to take this opportunity to welcome you to Jordan Creek Elementary School. You will find it to be an excellent school with sincere personnel willing to work with you to make our school the "best."
Parent involvement and support is encouraged. Students who know their parents are involved and communicate effectively with teachers usually do well in their schoolwork. Interest and enthusiasm are contagious. Please read the family handbook together and refer to it during the school year as needed.
It is our desire to provide each Jordan Creek student learning environments that are warm, caring, and educationally stimulating. We hope that students and parents alike will find Jordan Creek to be a school of which they can be proud and supportive.
